Bluestone Physician Services is pleased to announce our 2017 partner for fall Immunization clinics, CVS/Omnicare. Please review the frequently asked questions below to obtain further information on clinic sign up for the 2017 season.

What immunizations will be provided?

CVS/Omnicare will provide the following vaccinations.  Please note, with coordination of your clinic CVS/Omnicare will confirm the total number of immunizations needed for each type.

  • Influenza (Administer based on age)
    • Under 65 – Seasonal quadrivalent serum
    • Over 65 – Seasonal high dose serum
  • Pneumococcal (Administer what is appropriate for patient)
    • Prevnar 13
    • Prevnar 23
  • Zoster (Zostavax)
  • TDap (Boostrix)

How will the consent process work?

CVS/Omnicare will distribute their specific consent form.  This consent form must be signed in order for the patient to receive desired immunizations.  Bluestone will provide the consent form in a PDF format as well as an electronic format at  Families can access an immunization guide, which will highlight clinical recommendations from our Bluestone Clinical Quality team.
