It's Flu Shot Time!

The 2011-2012 Inactive Influenza Vaccine (flu shot) will be made available to all Bluestone patients.

Please review the Influenza Vaccine information sheet listed below. In order for this vaccination to be administered, please complete all information and sign the Consent Form. This form must be provided in person, by mail or fax to the Nurse at the patient's residence.

Flu Shot Information Sheet Contains information you need to know about the Inactivated Influenza Vaccine for 2011-2012, and provides information about who should get vaccinated, why, what the risks are, and who should not get vaccinated.
Vaccination Consent Form This is the form that will need to be completed and provided to the facility nurse.
