Todd Stivland, MD. is the owner and CEO of Bluestone Physician Services. With over 20 years of experience as a family physician, Dr. Stivland is dedicated to changing the way health care is delivered.
After founding Bluestone Physician Services in 2006, Dr. Stivland has grown it from a small start-up to the largest on-site provider for people in assisted living communities in the country. In Bluestone Physician Services, he has created an innovative health care model for frail chronically ill patients who need extensive medical and community services.
Dr. Stivland is active in state and national health care reform and has served on the Minnesota Governor’s Health Care Finance Task Force. He is an active speaker on medical models and clinical practices.

Sarah Keenan, RN, BSN
Chief Clinical Officer (CCO), President: Bluestone Integrated Care
Sarah Keenan has worked with seniors in the health care industry for over 20 years, providing community care expertise and implementing clinical programs serving over 10,000 people.
Sarah has been with Bluestone Physician Services since 2011 and provides leadership to the Bluestone Disability model of primary on-site patient care for people with disabilities. She designed, implemented and provided oversight to this model which improves the quality and delivery efficiency of primary care for people with disabilities living independently or in group home settings.
Sarah started out her career as a registered nurse. She became director of nursing for long-term care where she worked with seniors as one of the first Medicare coordinators. Keenan also worked as a care coordinator for seniors insured by Medica, one of Minnesota’s largest health insurers.
She has been published in medical home journals and speaks nationally on care coordination and preventative care in the Medicare population.

Chief Operating Officer (COO), President: Bluestone National, LLC
Tim Koehler is the president of Bluestone National, LLC. Tim is dedicated to expanding the Bluestone Physician Services model in the national health care market; improving the way medical care is delivered to vulnerable and high-risk patient populations.
Tim’s 18+ year tenure at UnitedHealth Group included roles in executive leadership, operations, finance, sales, and account management, acquisitions integration, new market strategy, personnel mentoring and development. He is most recently the former president of the Diabetes Prevention and Control Alliance (DCPA), a UnitedHealth Group organization.
Tim’s focus throughout his career has been improving the effectiveness and efficiency of healthcare, moving towards better and more sustainable systems which benefit both the individual patient and the community as a whole.

Chief Information Officer (CIO)
John Kraft has been Bluestone Physician Services chief information officer since 2011. He oversees all aspects of Bluestone’s data operations and technology infrastructure, including the electronic medical record systems and custom software systems for data analysis and reporting.
Kraft created Bluestone Bridge, a web-based platform that enhances communications and connections between health care providers, nursing staff, and family members.
Kraft has over 25 years of experience in a wide range of technology disciplines, which he combines with extensive experience in business and project management. He also owns SalesOrders.Net, a web hosting and application development company where he provides IT consulting, project management and other services to retail supply chain and health care entities.
Kraft was the chief software architect for Geacom, Inc., where he led software development for a handheld data acquisition device used by urgent care and emergency responders. He was also CIO of Wild Impact Marketing, a promotional products and marketing services company serving Fortune 1000 customers.
